This is a brief overview of the Surveys in OpenFIT.


1. Standardized Outcome Measurement Tools 

These are validated, widely used clinical and research tools.


Mental Health & Well-Being

  • ORS (Outcome Rating Scale) – A self-report tool used at the start of therapy sessions to measure a client’s subjective well-being across individual, relational, and social domains.
  • SRS (Session Rating Scale) – Assesses the strength of the therapeutic alliance at the end of a session, covering the relationship, goals, and methods used in therapy.
  • WSAS - (Work and Social Adjustment Scale – Adult) – Measures the impact of mental health difficulties on work, home life, and social interactions.
  • WSAS - (Work and Social Adjustment Scale – Child/Adolescent) – Assesses how psychological distress affects a young person’s academic, social, and daily life.
  • PHQ9 (Patient Health Questionnaire-9) – Screens for depression severity, widely used in mental health settings.
  • GAD7 (Generalized Anxiety Disorder-7) – Evaluates the severity of generalized anxiety disorder symptoms.
  • WHO5 (WHO-5 Well-Being Index) – A short, five-item questionnaire developed by the WHO to assess psychological well-being and screen for depression.



Refugee & Trauma Screening

  • PROTECTIONSURVEY (PROTECT Psychological Vulnerability Assessment) – Developed under the Protect-able project, this tool screens for PTSD risk and psychological distress in refugees.



Physical & Functional Health

  • FACIT-F (Functional Assessment of Chronic Illness Therapy – Fatigue Scale) – A validated measure assessing fatigue in chronic illness, commonly used in oncology and autoimmune diseases.



Treatment Effectiveness

  • EFFEKT-DATA - Voksen – A validated Scandinavian measure assessing the effectiveness of therapeutic interventions in adult populations.
  • EFFEKT-DATA - Barn – A child-focused version of the Effekt-Data tool evaluating treatment outcomes in pediatric clients.




2. Custom OpenFIT Surveys

These are custom-developed measures developed for agencies/health systems working with OpenFIT and used with their populations.

Mental Health & Social Care

  • SFH (Service Feedback & Health) Survey – A custom collaboration and service evaluation tool designed to measure how well social workers and case managers work with care providers, assessing communication, treatment effectiveness, and placement success.
  • PFRS (Post-Family Relationship Stress Survey) – Measures well-being after major family disruptions such as divorce or conflict.
  • MRS (Mental Resilience Scale) – Likely related to mental resilience and stress recovery, but its source is unclear.


Refugee & Social Well-Being

  • Family Integration Survey – Part of the Danish refugee integration model, measuring parenting, family relationships, and child well-being.
  • Social Integration Survey – Tracks employment, housing, community engagement, and well-being in refugees.


Violence Recovery

  • Violence Recovery Survey – Developed in Finland/Sweden, this tool measures trauma recovery and resilience in individuals affected by domestic, sexual, or honor-based violence.
